Permission Slips/Release Forms   Permission slips are required for all activities. Once you have completed a permission slip, it will be valid from January 1, 2012- March 1, 2013.   Signing a permission slip will allow your child to participate in ALL events throughout the year.   Please click on the link below, print out permission slip, complete it and return to the Children's Director.
